We actually talk a bit about the AFI and other 3rd trimester tests in my free prenatal class. What does amniotic fluid smell like and look like? A larger percentage (65%), of PROM cases presented before 24 h of drainage while the remaining 35% presented after 24 h of drainage. This is similar to what was obtained by Sharma et al. and Khan et al. in which 91.6% and 61.67%, respectively, presented before 24 h of drainage.[ 6, 23] Once that water is broken there is a chance of bacteria coming up the cervix from the vagina. That can cause an infection in the baby or the mom — called Chorioamnionitis– which can be a HUGE DEAL. It’s sort of easy to think that “maybe” your water being broken is not a big deal, but it really can be a big deal. If you get up and feel another gush of fluid, it likely means that your water is broken. The gush happens because it collects in your vaginal vault while you’re lying down. When you get up, it comes out.

Risks of PROM at term are related to serious neonatal consequences such as pre-term delivery, fetal distress, prolapsed cord, abruptio placentae and infection (2). pPROM accounts for 20% to 40% of PROM cases, and is associated with 20% to 50% of premature births. Complications of pPROM include infectious morbidity in the mother and fetus, pulmonary hypoplasia of the fetus, prolapse of the umbilical cord, development of fetal deformities, and postnatal endometritis (2). All of these consequences significantly increase risk of fetal and maternal morbidity and mortality. So, that’s the deal on an at home amniotic fluid test. The values for sensitivity, specificity, PPV, NPV, and accuracy of vaginal fluid urea level from this study was 18.8%, 95.3%, 80.0%, 54.0%, and 57.0%, respectively, while other studies were 90%, 79%, 83%, 87.5%, and 85%, respectively, by Kariman et al.,[ 26] 96% and 93%, respectively, for sensitivity and specificity of urea and 97% and 98%, respectively, for creatinine by Malchi et al.,[ 27] 100% for all the parameters by Hanfy[ 25] and Mohammed et al.[ 10]

If your water is broken, the fluid mixed together with estrogen will create a “fern-like” pattern under a microscope due to salt crystallization. A few drops of fluid will be placed on a microscope slide and observed under a microscope. Compared to the use of amniocentesis test strips, the amniocentesis performed by a gynecologist is very complex. In this procedure, the physician removes a small amount of amniotic fluid from the amniotic sac with the help of a hollow needle. This contains fetal cells that can be isolated and then multiplied in cell culture. Such a cell culture is important because only then is sufficient genetic material available to perform the necessary medical examinations. Amniocentesis looks at a sample of amniotic fluid. Amniotic fluid is a clear or pale yellow liquid that surrounds and protects an unborn baby throughout pregnancy. The fluid contains cells that provide important information about your unborn baby's health. Your health care provider will likely proceed to deliver the baby if the hospital has neonatal care available. According to Sanford Health, two-fifths of women at this stage will deliver the baby within a week. Many will deliver within 48 hours.However, you should bear in mind that the results provided by an amniotic fluid test strip are not always 100 percent reliable. This is because the amount of fluid is sometimes not sufficient to make a really reliable diagnosis. Therefore, if in doubt, visit your gynecologist immediately and have yourself examined accordingly. Alternatively, you can ask your midwife for advice. That being said, most often if you wonder if it’s broken, but instead put on a pad and watch it for 15-20 minutes to see if you’re still leaking. That should be fine.
